Recurrence of a pituitary adenoma

In this case, we are dealing with a recurrence of a pituitary tumor that has grown significantly upward and to the right. It is possible to resect the adenoma via a transnasal approach, but this method presents certain challenges. Using an endoscopic approach, we can advance laterally and remove as much of the tumor as possible. 

A transcranial approach would allow for more radical tumor removal, but it is associated with significant risks in this case. For this reason, I consider the transnasal method to be more appropriate. Any remaining tumor tissue in the lateral section of the cavernous sinus can be effectively treated with radiation therapy. In any case, tumor removal is necessary due to the disease's clinical presentation, particularly the deterioration of vision.

The MRI images are of good quality, but the examination was performed without a contrast agent, which is recommended in such cases.

The postoperative period proceeded without complications. However, postoperative images revealed residual tumor tissue in the cavernous sinus region. After a thorough discussion of the case at our neuro-oncology conference, we recommended that the patient undergo radiation therapy at a later date.
