MRI images of the brain show a mass lesion on the left side, extending from the thalamus to the midbrain, most likely an astrocytoma. After an additional MRI examination and careful surgical planning, I consider removal of the tumor feasible via a left temporal approach.
Parent’s Questions
We contacted several international clinics, where we were told that the tumor cannot be removed due to its difficult location. We were warned about the risk of serious complications after surgery — loss of motor function and cognitive impairment — and were advised to undergo radiation and chemotherapy instead. What is the likelihood of such consequences? What can we expect if we decide against surgery?
Doctor’s Answer
The tumor is indeed located in a difficult-to-access region of the brain, but after an additional MRI examination and careful preoperative planning, I consider its removal possible. I have experience performing similar surgeries. Moreover, I plan to perform the operation not from above, but through the left temporal region, under neuronavigation control, which significantly reduces the risks. It is a complex surgery, but it can be done. If the tumor is not removed, the child’s condition will rapidly deteriorate.

Treatment Performed
Surgical removal of the tumor was performed through a left temporal approach using MR-based computer navigation. An improvement in coordination of the right side of the body was observed. Postoperative MRI scans showed no residual tumor. The brain mass (WHO Grade I pilocytic astrocytoma) was completely removed. According to the tumor board, no additional chemotherapy or radiotherapy was required. The patient was discharged from the hospital in good condition on the 10th postoperative day.
