Reply from Onco-Urologist Prof. Thomas Otto

The patient presents with a testicular tumor. I recommend surgical exploration of the left testis with intraoperative histological examination. If malignancy is confirmed, a unilateral radical orchiectomy (removal of the left testis) and a biopsy of the right testis should be performed. In the event that the tumor proves to be malignant, further diagnostic procedures will be required: CT scan of the chest, abdomen, and pelvis. Based on the histological findings (seminoma, non-seminoma, or mixed germ cell tumor) and the disease stage, we will provide a detailed recommendation for the next steps of treatment — active surveillance, chemotherapy, and/or radiotherapy.

Patient Questions

  • Will an urgent histological examination be performed during surgery?
  • What diagnostic procedures will be carried out before the operation?
  • If postoperative treatment is needed, can it be done at your clinic?

Physician Answers

  • Yes, we can perform intraoperative frozen-section histological examination, and I plan to do so. Prior to surgery, the patient will undergo physical examination and laboratory testing.
  • If a malignant tumor is found, we will review all available laboratory results and imaging studies (DICOM format) from the patient’s local hospital and then decide which additional diagnostic procedures are necessary (these costs are not included in the initial estimate). Following this evaluation, we will propose a personalized treatment plan (chemotherapy and/or radiotherapy) and provide a corresponding cost estimate.
  • If required, further treatment can also be carried out at our clinic.

Estimated treatment cost: €4.329
