Bone Metastases in Breast Cancer

Diagnosis

Metastases to the ilium and spine. Condition post-resection of invasive ductal carcinoma of the breast 8 years ago, followed by postoperative radiation therapy and hormone therapy.

Patient:

Female, 55 years old, Kazakhstan

Physician:

Dr. Jörg Fahlbrede, Chief Physician of the Department of Gynecology and Gynecological Oncology at Agaplesion Bethesda Hospital

Patient Query:

I have been prescribed a chemotherapy regimen consisting of docetaxel, doxorubicin, cyclophosphamide, and zoledronic acid, along with palliative radiation therapy. Additionally, hyperthermia and hyperbaric oxygen therapy have been recommended to me.

I experienced severe side effects during the first cycle of chemotherapy. I would like to ask if there are alternative treatment options available for my condition in Germany?

Response from Dr. Jörg Fahlbrede:

The proposed therapy lacks modern scientific justification. In Germany, administering this regimen under these circumstances would be considered medical malpractice.

The standard treatment protocol in Germany for this clinical scenario is structured as follows:

  • Targeted Hormone & Cyclin-Dependent Kinase Therapy: CDK4/6 inhibitors combined with either fulvestrant or letrozole (for postmenopausal patients), administered daily for 3 weeks followed by a 1-week rest period (or as a single injection every 4 weeks).
  • Bone Stabilization: Bisphosphonate therapy or RANK ligand inhibitors (RANKL inhibitors) to strengthen bone structure (one injection every 4 weeks).
  • Radiation Therapy: Indicated only in cases of severe pain or a high risk of pathological fractures.
  • Second-Line Targeted Therapy: If the initial therapy proves ineffective (which is unlikely), a combination of everolimus and exemestane (oral tablets) is prescribed.
  • Targeted Therapy for Specific Mutations: In the presence of a PI3K-α gene mutation, alpelisib (oral tablets) is utilized if necessary.
  • Chemotherapy Restraint: Systemic chemotherapy is no longer indicated for hormone receptor-positive tumors.

Follow-up Patient Query:

May I clarify whether you were referring to the chemotherapy combination of docetaxel, doxorubicin, cyclophosphamide, and zoledronic acid as medical malpractice, or did you mean the hyperthermia and hyperbaric oxygenation?

Response from Dr. Jörg Fahlbrede:

Yes, that is correct — I was referring specifically to that chemotherapy regimen. Furthermore, I consider the use of hyperthermia and hyperbaric oxygen therapy to be clinically questionable in this context.
