Hip Osteoarthritis Treatment Consultation Case Study: Total Hip Arthroplasty (Charité, Berlin)
Diagnosis:
Left Hip Joint: Stage IV severe osteoarthritis (end-stage hip osteoarthritis / coxarthrosis). Mushroom-shaped femoral head deformity, extensive cystic degeneration, bone marrow edema extending into the femoral neck, synovitis, and iliopsoas bursitis.
Right Hip Joint: Stage II mild-to-moderate osteoarthritis with subchondral cysts, sclerosis, and mild bone marrow edema.
Patient:
Male, 69 years old, Belarus
Physician:
Prof. Dr. Carsten Perka, Director of the Center for Musculoskeletal Surgery, Charité – Universitätsmedizin Berlin
Key Diagnostic Findings
Left Hip (Stage IV Coxarthrosis): MRI shows severe advanced osteoarthritis with a mushroom-shaped collapse/deformity of the femoral head. Inhomogeneous MRI signal due to multiple cystic areas and severe bone marrow edema extending into the femoral neck and intertrochanteric region. Acetabulum shows subchondral cysts, bone marrow edema, and superior acetabular rim osteophytes. Moderate joint effusion accompanied by active synovitis and iliopsoas bursitis (13 mm fluid collection along the iliopsoas tendon).
Right Hip (Stage II Coxarthrosis): MRI indicates early-to-moderate degenerative changes. Femoral head maintains a round contour with localized medial subchondral bone marrow edema and minor osteophytes. Acetabular roof sclerosis and mild joint effusion are present.
Patient Symptoms & Questions
Clinical Symptoms:
- Left Hip: Severe, constant hip pain, marked joint stiffness, severe range-of-motion restriction, weight-bearing impairment, and pronounced limp.
- Right Hip: Moderate pain triggered by prolonged walking and physical exertion.
Patient Inquiries:
- Hip Resurfacing (McMinn / BHR Method): "Is hip resurfacing (Birmingham Hip Resurfacing / McMinn technique) feasible in my case to preserve as much bone as possible?"
- Minimally Invasive Surgery: "Do you utilize minimally invasive hip replacement techniques to minimize tissue damage, surgical incisions, and recovery time?"
- Management of the Right Hip: "Should my right hip (Stage II) be operated on, or can it be managed with conservative treatments (injections, medication, physical therapy)? Will it deteriorate faster when bearing extra load during recovery?"
- Hospitalization & Recovery: "How long will I stay in the hospital? Will I require post-operative intensive care (ICU), or will I go straight to a standard ward?"
Surgical Opinion & Medical Recommendations:
- "The patient's severe symptoms and distress are fully understandable. This is a clear case of advanced end-stage (Stage IV) osteoarthritis of the left hip. Due to the extensive cystic degeneration and bone marrow edema in the femoral head and neck, I strongly advise against hip resurfacing (McMinn/BHR). While technically feasible, performing resurfacing in this specific anatomical condition carries a significantly elevated risk of femoral neck fracture. Therefore, I recommend a uncemented primary total hip replacement (zementfreie Hüfttotalendoprothese). I would be pleased to discuss all technical details during a personal clinical consultation.
- For total hip arthroplasty, we routinely employ minimally invasive, muscle-sparing surgical approaches to minimize soft-tissue trauma and accelerate post-operative mobilization.
- Regarding the right hip, I strongly recommend conservative management (medication, physical therapy, intra-articular injections). Once the left hip is fully rehabilitated and capable of bearing full weight, the load burden on the right hip will decrease, leading to symptomatic improvement.
- Hospitalization typically requires 4 to 5 inpatient days. Pre-operative preparation includes routine laboratory testing, X-rays, and an anesthesiology clearance. An intensive care unit (ICU/ITS) stay is not required."
